Supported Accommodation

Unified Care Support Services provides a number of accommodation options for NDIS participants who require specialist housing solutions, our services and facilities are latest and properly designed to ensure complete satisfaction

According to your needs and requirement the following housing options are available

Supported Independent Living [SIL]:  Unified Care support provides unparalleled Supported Independent Living (SIL) service in Melbourne. Our SIL service is available to people with disabilities to help them live as independently as possible. You are eligible to receive SIL support service if you live in a home with other people. This helps to cover the cost of the support staff in the house.

Unified Care’s SIL option offers you:

  • Assistance with personal care and domestic cleaning
  • Assistance to participate in social and community activities.
  • Assists the residents organise household tasks like preparing food, cleaning etc.
  • Assistance to learn new skills
  • Manages and attend appointments with you

Independent Living Options [ILO]: Refers to the type of service where you can choose the accommodation you prefer and the supports you will need. This two-stage service lets you explore and design the supports you need and is an alternative to SIL and to group home services. You can also choose to share your home with friends or housemates too.

Medium Term Accommodation [MTA]: MTA is an intermediary support service where NDIS participants are preparing to move into a more permanent home. And awaiting a confirmed long-term accommodation vacancy.

It can last up to 90 days.

Short Term Accommodation [STA] : STA support service is funded for a short stay away from your usual accommodation. You can choose to live by yourself or in a group setting

It is provided for up to 14 days at a time

Respite Care: It is a support service that is available to you or your family when you need a break from your usual routine or in case of an emergency. It is time-limited and is available for a certain number of days in a year

Support Coordination

Unified Care Support Services have the best Support Coordination Team and our Support Coordination program helps you to identify and purchase the supports and services you need to achieve your goals We will work side by side with you to strengthen your ability to coordinate and utilize all the supports provided by your NDIS plan so that you can become an active member of the community.

Support connection – This support service builds your ability to connect with informal, community and funded supports – Generally provided by the LACs

Support coordination – This support service includes a support coordinator who will work with you to ensure coordination of various supports to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and participate in your community.

Specialist support coordination – This is a higher level of support coordination for people whose situations are more complex and who need specialist support in getting rid of any barriers that may prevent access to your NDIS plan.

Assistance with Daily Living Life Skills

Unified Care’s Daily Living Life Skills program is designed to meet the needs of individuals and promote personal growth, independence and improving functional capacity. This program is customised for the individual to receive specific skills and social training so they can achieve their goals and aspirations or work towards them. Unified Care’s main focus is to promote independence by providing the necessary support and improve the confidence level by building on those meaningful and necessary life skills, program will be designed with the participant’s input and will be reviewed periodically.

This program is ideal for someone who wants to improve their Quality of life, Develop or improve daily living skills and social skills, manage their emotional and personal wellbeing, grab the opportunity to maintain their still or learn new skills to become more independent in their lives.

Unified Care’s Support Team will help you develop or maintain your daily living skills, at home and whilst you are out and about in the community.

Assistance with Daily Personal Activities

Assistance with Daily Personal Activities is a service that Unified Care Support Services offers to assist you with daily personal activities like showering, bathing, oral hygiene etc. Our dedicated and respectful staff are proficient in all aspects of personal care and we make sure that you look and feel your best. We work with you to ensure that you develop a regular personal hygiene routine, which is the most effective way to improve and maintain your health and improve your strength, balance and mobility.

Our services are available to you across a variety of settings- in the comfort of your own home, when you are living with family, when you are undertaking social, recreational, education or employment activities or when you are on vacation. Our personal care supports usually assist you with a number of goals, rather than one specific goal.We believe that maximising your independence and functional skills is an integral part of achieving your goals.

Our services include but are not limited to:

  • Assistance with personal hygiene [ oral hygiene, bathing, dressing & personal grooming ]
  • Assistance with toileting
  • Assistance with eating and drinking
  • Assistance with mobility and transferring ex: moving into and out of bed and moving on or off the toilet
  • Attending appointments
  • Basic first aid
  • Bladder and Bowel Management and Menstrual care

Household Tasks

Unified Care Support Services provide support to enable you to live as independently as possible. We ensure that you live in a safe, healthy environment where you are comfortable and independent at all times.

Unified Care Support focuses on skill development, our experienced staff will assist you to learn new skills or to maintain your skills to promote independence and work towards achieving your short-term / long-term goals. If you have Core supports funding in your NDIS budget, there is flexibility for choosing supports that meet your individual needs with household tasks.

We assist you with the following tasks:

  • Grocery shopping and meal preparation
  • Personal hygiene: includes showering, dressing and grooming
  • Meal Preparation
  • Kitchen cleaning including sinks, taps and microwaves
  • Budgeting
  • Vacuuming and mopping floors
  • Dusting and wiping surfaces
  • Changing linen
  • Laundry and ironing
  • Lawn mowing and yard maintenance
  • Cleaning toilets, showers and baths

Community Participation

Unified Care Support provides NDIS Services based in Melbourne, recreation and leisure activities are very important to every individual, being connected to our communities and participating in social activities contributes to quality of life, general wellbeing and also it helps with skill development and competencies.

 Unified Care Support is dedicated to breaking down any barriers you may face to participate fully in society. We encourage and support you to go out and about, meet new people and try new things.  We are here for you every step of the way, as you find your niche in society.
